Patients who are on a pre-paid care plan can use the form below to submit supplement orders and receive their 10% off supplements.
Supplements do not come with the supplement pumps.

Shipping Details:
Shipped Orders: Allow 2-3 business days for processing. If any items are out of stock, we will contact you to inform you of the expected timeline. We typically wait until all items are in stock before shipping to ensure your order is complete.
If we are out of a supplement when you submit your order, we will contact you for further instruction.

In-Person Pickup Orders: Orders for pickup will be ready within 2-3 hours after your request. Our pickup hours are:
Monday: 8:30 AM – 12:00 PM, 1:00 PM – 5:30 PM
Tuesday: 12:00 PM – 5:30 PM
Wednesday: 8:30 AM – 12:00 PM, 1:00 PM – 5:30 PM
Thursday: 1:00 PM – 5:30 PM
Friday: 8:30 AM – 12:00 PM
